Did You Know?

Congratulations to Collier Health Services & Bond Community Health Center! CHCA assisted both organizations in the preparation of applications for grant funding awarded in August and September, 2009.

Enabling Services Expansion: CHS outreach workers will complete home visits to new mothers within 48-hours of delivery, evaluating the condition of both mother and infant to determine their immediate needs, helping the mother schedule follow-up appointments for herself and her baby, and administering the Postpartum Depression Screening Scale.

Behavioral Health Services Expansion: CHS will expand screening for developmental challenges - such as Autism and ADHD - into routine pediatric visits.

Bond Community Health Center in Tallahassee serves those living with HIV/AIDS across an eight-county service area under the Ryan White Part C-EIS program.  With expansion funds, Bond will bring both Infectious Disease Specialty Care and Nutrition Counseling within the health center; provide additional resources for Case Management and Pre/Post Test Counseling; and ensure transportation to medical social services, and support group activities for those with mobility impairment requiring wheelchair accommodation.
